Checklists, forms and permits your team will actually use

Overview

Every workplace needs more than policies and procedures. The day to day operation of a safe workplace depends on the forms, checklists, and permits your workers use every single day. These are the documents that capture what actually happens on site — who was inducted, what hazards were identified, what equipment was inspected, what incidents occurred, and what high risk work was authorised before it started.

At Local Safety Solutions we design and build every form, checklist, and permit your business needs. Everything is tailored to your business, branded with your company name and logo, and built in a format your workers can complete quickly and correctly on site without a safety advisor present.

Generic downloaded forms signal that safety is an afterthought in your business. Custom branded forms signal a professional, compliant operation that takes safety seriously. When a principal contractor or WorkSafe inspector picks up one of your forms, the first impression matters. Ours hold up.

We cover the full range of safety forms, checklists, and permits. For forms we produce an Incident Report Form capturing all details of a workplace incident, near miss, or injury, a Hazard Report Form allowing workers to report a hazard before it causes harm, a Near Miss Report Form recording events that nearly caused harm and what needs to change, a Return to Work Form documenting agreed duties, hours, restrictions, and medical clearance for injured workers, and a Visitor Sign-in and Induction Form combining site sign-in with safety acknowledgement.

For checklists we produce a Take 5 Pre-Task Checklist for daily use before any task begins, a Plant Pre-Start Inspection Checklist completed before operating any vehicle or piece of plant, a Workplace Inspection Checklist for regular site safety inspections, a Toolbox Talk Record Form capturing attendees and topics covered, an Emergency Drill Record Form documenting that drills were completed and what improvements were identified, a Site Induction Checklist covering every topic that must be addressed before a worker starts on site, and a SWMS Worker Sign-off Sheet as legally required before high risk construction work commences.

For permits we produce a Hot Work Permit required before any welding, grinding, cutting, or open flame work in areas where flammable materials may be present, a Confined Space Entry Permit required before any worker enters a confined space where atmospheric hazards may exist, an Excavation Permit required before excavation begins in areas where underground services may be present, an Electrical Isolation Permit covering lockout tagout requirements before any work on or near electrical equipment that must be de-energised, a Working at Heights Permit for high risk height work where edge protection alone is not sufficient, a Ground Disturbance Permit required before any digging or ground penetration where underground infrastructure may be affected, and a Chemical Work Permit for work involving chemicals that pose an acute risk.

If you need a form, checklist, or permit that is not on this list we can build it. If you are not sure what you need we will tell you.

Who needs this

For every business with workers

Every business with workers needs safety forms, checklists, and permits in place. If your workers perform high risk work, operate plant, use chemicals, or work on construction sites then you need the right documents to capture what is happening on site every day.

If a WorkSafe inspector arrives and asks for your incident register and there are no incident report forms to populate it with, your system has a gap. If a principal contractor asks for evidence that your workers were inducted on site and you have no signed induction forms, you cannot prove it happened. If high risk work begins without a completed and authorised permit, you are in breach of your legal obligations.

Local Safety Solutions makes sure your business has every form, checklist, and permit it needs — built properly, branded professionally, and ready to use from day one.
